Enter the student's full legal/full name with the correct spelling as it appears in the student's Maktab records.
Please double-check:
First name
Middle name, if applicable
Last name
Spelling and capitalization
Enter the student's complete and correct date of birth.
Please verify the month, day, and year before submitting the form.
The Student Number is required to correctly match your payment and enrollment information to your child's QuickSchools student record. You can find the Student Number in your child's QuickSchools student profile, in the field directly above the Birth Date.
Accurate student information is important because the payment and enrollment records will be matched to the student's Maktab account.

If you are enrolling more than one child in the Maktab program, please select the total number of children you are enrolling on this page.
You should then provide the full name and date of birth for each child in the forms provided.
Please submit all children together as one order.
Do not create separate orders for each child if you are enrolling multiple children at the same time.
Before submitting, please review the information for each student carefully to make sure all names and dates of birth are correct.
